University Health Services
Yale University Health Services (YUHS) is located on campus at 17 Hillhouse Avenue.
YUHS offers a wide variety of health care services for Yale students. Services include student medicine (primary care), sports medicine/athletic care, mental health and counseling, a pharmacy, a twenty-three-bed infirmary (the Inpatient Care Facility), a twenty-four-hour Urgent Care Department, and many specialty clinics such as Allergy, Dermatology, Orthopedics, etc.
YUHS also includes the Yale Health Plan (YHP), a health care coverage option that coordinates and provides payment for services outlined above as well as for emergency treatment, off-site specialty services, inpatient hospital care, and other ancillary services.
YUHS's services are comprehensively described in the Yale Health Plan Student Handbook, available through the YHP Member Services Department, 17 Hillhouse Avenue (432-0246) or on line (www.yale.edu/yhp).
Yale degree candidates enrolled at least half time are eligible to receive YHP Basic Coverage at no charge and are enrolled automatically for these services. YHP Basic includes preventive health and medical services in the departments of Student Medicine, Health Education, Mental Hygiene, Nutrition, and Gynecology. In addition, through the Urgent Care Department, treatment for urgent medical problems can be obtained twenty-four hours a day. Students who need more acute care receive services in the twenty-three-bed Inpatient Care Facility.
For students entering Yale for the first time, readmitted students, and students returning from leaves of absence who have not been covered during their leaves, coverage for YHP Basic and the use of other YUHS services begins on the day the dormitories officially open. Subsequently, coverage for YHP Basic is effective from September 1 through August 31. A student who is enrolled for the fall term only is eligible to use services through January 31; a student enrolled for the spring term only is eligible to use services through August 31.
Eli Whitney Students are not eligible for YHP Basic but may purchase the YHP Affiliate Coverage or use services on a fee-for-service basis. Students who wish to be seen on a fee-for-service basis must register with the YHP Member Services Department.
Students in the Special Student nondegree program are not eligible for YHP Basic but may join as members of the YHP Billed Associates Program or use services on a fee-for-service basis. Students who wish to be seen on a fee-for-service basis must register with the YHP Member Services Department.
All students are welcome to use specialty and ancillary services at YUHS. Upon referral, YHP will cover the cost of these services if the student has YHP Hospitalization/Specialty Coverage. (See section C, "Health coverage enrollment.") If the student has an alternate insurance plan, YHP will assist in submitting claims for specialty and ancillary services to the other plan and will bill the student's account with Student Financial Services for noncovered charges and unpaid balances.
1. Withdrawal. A student who withdraws from the University on or before the tenth day of a term will not be held responsible for the tuition fee for that term and as a result will not be eligible for any YHP benefits. Under such circumstances, the student will be refunded the premium fees paid for YHP Hospitalization/Specialty Coverage and, if applicable, the YHP Prescription Plus Coverage. The student's YHP membership will be terminated retroactively to the start of the term and any services rendered and/or claims paid will be billed to the student on a fee-for-service basis.
A student who withdraws from the University after the first ten days will be covered by YHP for 30 days following the date of withdrawal, or to the last day of the term, whichever comes first. A student who is hospitalized on the date of withdrawal or during the 30-day extension and has the YHP Hospitalization/Specialty Coverage will have coverage until discharged from the hospital up to the benefit limitation for that illness. YHP premiums are not refunded and will not be pro-rated. Students who withdraw are not eligible for YHP Affiliate Coverage.
2. Leaves of Absence. Students who are granted leaves of absence are eligible to purchase the YHP Affiliate (Self-Pay) Coverage during the term of the leave. In order to secure continuous coverage through YHP, students must request enrollment in the YHP Affiliate (Self-Pay) Coverage prior to the beginning of the term during which the leave is taken. Coverage is not automatic. Further information and application forms may be obtained from the YHP Member Services Department (432-0246) or on line (www.yale.edu/yhp).
The University requires all full-time Yale degree candidate students to obtain adequate hospitalization coverage while enrolled at the University. Students may choose the YHP Hospitalization/Specialty Coverage or elect to waive the coverage if they are covered for hospitalization under another policy, such as a parent's policy. The waiver must be renewed annually by September 15 for the full year or the fall term, or by January 31 for the spring term.

Students who are enrolled during both the fall and spring terms will be covered from September 1 through August 31. A student whose first term of enrollment in the academic year is the spring term will be charged a fee for coverage from February 1 through August 31.

a. Waiving the YHP Hospitalization/Specialty Coverage. Students are permitted to waive the YHP Hospitalization/Specialty Coverage by completing a waiver form that includes proof of alternate coverage. Waiver forms are available from the YHP Member Services Department (432-0246) or on line (www.yale.edu/yhp). It is the student's responsibility to report any changes in alternate coverage to the YHP Member Services Department. Students and their parents who are considering waiving the YHP Hospitalization/Specialty Coverage are encouraged to review their present hospitalization coverage and to compare its benefits to those available under the YHP plan. Please note: students who waive the YHP Hospitalization/Specialty Coverage will not be eligible for YHP Prescription Plus Coverage, described in section C.2.)

This plan has been designed to provide coverage for some types of expenses, such as prescription drugs, not covered under the YHP Hospitalization/Specialty Coverage. Students who are enrolled during both the fall and spring terms will be billed each term and will be covered from September 1 through August 31. A student whose first term of enrollment in the academic year is the spring term will be charged a fee for coverage from February 1 through August 31.

Students may enroll their spouse or same-gender domestic partner and dependent children under the age of nineteen in the Yale Health Plan by annually filing an application with the YHP Member Services Department and by paying the additional premium each term. The student two-person plan is available for one dependent, either spouse/partner or child. The student family plan is for a student's family of three or more persons. These plans include coverage for YHP Basic Coverage and for the benefits covered under the YHP Hospitalization/Specialty Coverage. 4. YHP Affiliate Coverage. Students on leaves of absence, students on a Year or Term Abroad, and Eli Whitney Students who do not meet the requirements for automatic enrollment in the Yale Health Plan are eligible to purchase YHP Affiliate Coverage or to register as fee-for-service participants. YHP Affiliate Coverage includes the services of YHP Basic Coverage and YHP Hospitalization/Specialty Coverage.

D. Medical/immunization requirements
Each new student is required to supply the Yale University Health Services (YUHS) with a complete health history, the results of a physical examination, a complete immunization record, and a completed Authorization for Medical Care and Treatment permission waiver as required by the State of Connecticut for students under eighteen years of age. Forms are mailed to all incoming students during the summer before their arrival at Yale and must be completed and returned to YUHS prior to matriculation. Subsequent medical examinations after entrance may be required of any student at the discretion of the medical staff.
Students will not be allowed to matriculate without first providing satisfactory evidence of protection against rubeola (measles) and rubella (German measles). Two doses of rubeola and one dose of rubella vaccine are required for newly matriculated students. Rubeola dose 1 must have been given after January 1, 1969, and after the student's first birthday; dose 2 must have been given after January 1, 1980. Rubella vaccine must have been given after January 1, 1969, and after the student's first birthday. Complete details of this requirement are stipulated in the health history forms.
As of September 2002, all students living in on-campus housing must, by law, be vaccinated against meningococcal disease. All returning students who plan to live in University housing must be immunized or show proof of immunization within the last five years. Students who are not compliant with this law will not be permitted to register for classes or move into the dormitories. Please note that the State of Connecticut does not require this vaccine for students who elect to reside off campus.
Failure to obtain the required immunizations before matriculation will result in the student being placed on a registration hold. The cost of providing any of these immunizations will be charged to a student's account with Student Financial Services.
