Our clinicians – physicians, nurse practitioners, nurse midwives, physician associates and others – are committed to a team approach to health care. Our entire mission is to keep you well, and to provide the highest quality care when you are ill.

  • As a member of YHP, you have access to preventive, diagnostic and treatment services both at YHP and through a network of clinicians and services at Yale New Haven Hospital, Yale University School of Medicine, and other area facilities.

Illness doesn’t always happen at the most convenient time. Figuring out how to make decisions about specialty care or hospitalization can feel overwhelming. As a member of YHP, you can feel secure knowing that help is available 24 hours a day, 7 days a week.

  • The Urgent Care Department is open 24 hours a day, 365 days a year.
  • Our Care Coordination Department is available to assist with coordination of care between your primary care clinician and specialty care and/or hospitalization.
  • Our Inpatient Care Facility (ICF) offers inpatient evaluation and treatment for a wide range of conditions. It also provides transitional care after hospital stays and certain outpatient services requiring skilled care but not hospitalization.
  • Access to Healthwise, a self-care assessment tool to be used as a practical guide for deciding when you should see a doctor and which home treatments are best. You have the right to accurate information about your health conditions and treatment options. Medical decisions made with your informed input are most likely to yield the best results for you and your family. Every person, with the benefit of good information, can improve the quality and the results of his or her health care.
