QUESTIONS ABOUT SID NUMBERS
Please include your 9-digit System
Identification Number (SID) on the envelope that is
to be returned with your photos to be used in creating
your student ID card.
For those of you who have
already sent in your photos without the SID, we will
process your ID card using other information. Please
do not contact the ID Center to find out if your packet
has been received. If there is a problem with your photo,
we will notify you via the email address you provided.

General
Identification
Your ID card identifies you as a member
of the Yale University community. You should carry it at all
times while you are on campus and present it, upon request,
to University officials whose assigned responsibilities authorize
them to seek proper identification.
Caring for your ID Card
Please treat your ID card with care. Holes,
creases, and improperly applied validation stickers will make
the card inoperable. Do not bend the card or leave it in direct
sunlight, near other sources of heat, or in contact with magnetic
fields.

Features
Your photo, name,
signature, birth date (undergraduates only), card issue date,
university personal identifier (UPI), system identification
number (SID, Banner account holders only) and bar code are
on the front of the ID card. The bar code is used for identification
by Yale libraries.
One magnetic stripe
on the back provides access to campus buildings and to Dining
Services. The second, smaller, magnetic stripe on the back
is used by laundry facilities in University housing.
Undergraduates and others requiring
access to secured areas, such as residential college gates,
have IDs that contain a proximity antenna, which opens the
locks in those areas.
New ID Cards (First
Time Card Bearers)
The University requires one of these forms
of identification when obtaining a Yale University ID card
at the ID Center. NOTE: Name on government authorized ID cards
must match the name in Yale's database (no nicknames).

Replacement
cards
Lost, stolen or
damaged ID cards should be reported immediately to the University
Security Programs Department at 100 Church Street South, (203)
785-5555.
Replacement cards
are available at the ID Card Center during normal business
hours. During non-business hours, temporary ID cards are available
at the University Police Department located at Phelps Gate.
To obtain a replacement ID card, you
must go in person to the ID Center. For security reasons,
some other form of photo identification, such as a driver's
license or passport, is required (see list above).
When an ID card is lost, stolen, or no
longer functions, the ID Center issues a replacement card
with the photograph on record.
Malfunctioning
ID cards that are returned to the ID Center are replaced at
no charge. Lost, stolen, or deliberately damaged cards are
replaced at a fee of $20 or $5, depending on card type (proximity
or standard).
