Q Who is entitled
to a Yale ID card?
A All students,
full-time employees, most associates,
fellows, spouses and same sex civil
union partners of students and employees
are entitled to Yale ID cards.
Casual/Temporary employees are entitled
to ID cards only if their supervisors
request them through the appropriate
process.
Q
I am an incoming student. How do I get
my Yale ID card?
A A few months prior to
your matriculation at Yale, you will be
asked to submit a photograph for your
Yale ID card. Please go to www.yale.edu/idphotoupload
for additional details regarding photo
requirements and electronic
submission. During registration,
the Office of the Registrar of your
school will arrange for distribution of
your ID card.

I am an incoming student but did not
submit my photo to the ID Center. The
Registrar of my school does not have my
ID card. How do I get an ID card?
A You must obtain an
Authorization to Issue ID Card form from
the Registrar of your Yale school. Go to
the ID Center with this form and a
picture identification to get your ID
card.
Q
I am a new employee. How do I get my ID
card?
A Your department must
have processed your information and your
records must be in the Human Resources
database, before you can obtain an ID
card. Check that your department has
done this. Then go to the Yale ID Center
to obtain an ID card.
Q
Can my spouse or same sex civil union
partner obtain a Yale ID card, if I am a
Yale employee or student?
A Yes. These are
affiliate ID cards. To obtain an
affiliate card, the spouse or same sex
civil union partner must bring the
required documentation and
identification to the Yale ID Center as
indicated below:

Affiliates - Yale College,
Graduate & Professional Schools
(except Medical School*)
1. Photo ID of card recipient and
2. Spouse or same sex civil union
partner's Yale ID card or ID number and
3. Authorization to Issue ID Card form
(issued by Office of the Registrar of
appropriate Yale school after providing
the appropriate information listed below
under marital or civil union status
documentation to the registrar).

*Medical School students - Affiliate ID
cards will be issued at Medical School
ID center. Please call (203)
785-4286 for information regarding
documentation.
Employee
and Associate Affiliates

Q
What should I do if my ID card is lost
or stolen?
A You should
immediately report a lost or stolen ID
card to the Yale University Security
Programs Department (203-785-5555).
Replacement cards are available at the
ID Center. During non-business hours,
temporary ID cards are available by
calling 5-5555 from any campus blue
phone
Q Is there a
charge for replacing lost or stolen ID
cards?
A Yes. Lost proximity
cards are subject to a replacement
charge of $20. All other cards cost $5
to replace.
