Frederick
Flintstone, PhD, prominent professor of paleontology at Bedrock
University, has been invited to Yale University by the chair
of your department to speak at a colloquium on August 29,2001.
He will be spending one night and Yale will be paying him
honoraria for his speaking engagement as well as reimbursing
him for all his travel expenses (airfare, hotel and per diem).
He has written you, Betty Rubble, Assistant Administrator
in the Paleontology Department, the following e-mail and left
you the following phone message:

Dear
Ms. Rubble,
I am delighted to accept Prof. Rubble's invitation to present
at your colloquium next month as I have never before visited
Yale University. I understand that you need some information
from me to take care of the honoraria of $200 and to reimburse
for my travel expenses. The airfare was $350.00 and the
hotel was $100.00. I will send you my receipts through the
mail as you requested. My home address is 100 Stegosaurus
Drive, Bedrock, MA 12345. I prefer to have my check sent
to that address rather than to the university. Also, because
e-mail is pre-historically unsecure, I prefer to telephone
you with my Social Security number. I know there was some
question, so I also want to confirm that I am a US citizen.

ensure that he later receives his honoraria and reimbursement
in a timely fashion, Betty immediately completes a Vendor
Setup Request (VSR) form and then she completes a Check Request
(CR). She then emails the form to George Slate, Business Manager
of Paleontology for his authorization. FYI, Betty's phone
number is X2-2222 and her fax is X2-1111. George's phone number
