OISS OPT Request Form signed by your dean, department chairperson, or academic adviser. Pick an OPT start date: For post-completion OPT, your start date can be any day after your end date through the 60 day grace period. Be sure to pick a work day (i.e., Monday) as you will not be able to begin work until the date you pick.
If you do not yet have a job offer, you will need to estimate what start date will work best for you. For those graduating on May 20, 2013, the latest date you could choose would be July 20, 2013. Pick an OPT end date: If you have not used OPT prior to graduating, your end date will be 365 days after your start date. For example, if your start date is June 10, 2012, your end date will be June 9, 2013. If you have used pre-completion OPT, you will need to deduct that time from the allowable 12 months. |
