Do not enter punctuation, i.e., enter $10,000 as 10000.

Enter the total amounts of funding the person will receive. For example, if a scholar will receive 30,000 dollars per year for a 2-year appointment, you would enter 60000.

If the scholar will receive a Yale paycheck, please provide that amount in the Yale University Funds field.

For funds that are not from Yale, enter amounts in the appropriate fields (personal, home government, etc…)  Do not enter an explanation if personal funds.

Be sure to answer the required Yale Health Plan question.

Special attachment requirement:
Non-Yale funding requires that the scholar/department submit evidence of the funding noting the amount and the duration. This evidence should be written in English. See Attaching Documents for more information.
