Special requirement: ISD requires an e-mail address for the Local and Permanent fields.

Local
Address

The address the scholar will reside at while at Yale.

• Click the edit button.
• Please select Yes under "Is this a current address?"
• Please select No under "Use Department Address."
• If the scholar has entered an address outside the U.S., please override their entry and type in the address:
  421 Temple Street, New Haven, CT, 06511.
• If you are filling out this information on behalf of the scholar, enter the address:
  421 Temple Street, New Haven, CT, 06511.
• Please remember to provide the scholar's e-mail address.
• Be sure to click save.

Permanent
Address

The scholar's home address outside the U.S.

• Click the edit button.
• Select No from the Current Address drop-down menu.
• Enter scholar’s permanent address outside the U.S.
• Please remember to provide the scholar’s e-mail address.
• Be sure to click save.

Mailing
Address

If the immigration document should be sent to an address other than the Permanent Address, enter a Mailing Address here.

• Click the edit button.
• Select No from the Current Address drop-down menu.
• If the scholar wants their immigration documents sent to an address other than the permanent address, enter a mailing address here.
• If the mailing address is in the U.S., leave the Country dropdown blank.
• Please remember to provide the scholar’s e-mail address.
• Be sure to click save.
