When emailing waste forms to OEHS, please indicate the waste type in the subject line of your email (i.e. Radioactive Waste, Biological Waste, Universal Waste, Chemical Waste). This will allow for faster processing of your request.

Periodic changes are made to our waste forms. Please bookmark this page to be sure you are using the most recently updated form.

We encourage you to fax or email your requests, as this will increase the efficiency of the process. However, if you must phone in your request, you may call us at 785-3551. Hours for the request phone line are Monday through Friday from 7:30am to 4:00pm. As always, please allow 3-5 working days for pickup and delivery.
