I never had a problem with 5.* rpms not working under 6.*. The
compatability libraries appear to work pretty well.
I have had problems with the fact that all the new RPMS are made only
for 6.* and so I end up having to go get the sources and recompile it
for my 5.2 machine.
For me, the 5.* to 6.0 upgrades went without a hitch. (Well,... latex
changed in some subtle ways, some of which were fixed by a subsequent
update, but I find it really annoying that latex now comes by default
set up for A4 paper instead of letter paper. I keep having to fix
that...)
Mike Fischer reported some difficulty on his upgrade.
I have had no trouble with the off-the-shelf kernel on any of my
machines.

Conveniently enough, Red Hat has instructions on making limited
updates to a 5.2 installation in order to make it kernel-2.2 ready:

And I'd recommend compiling the kernel yourself rather than
rpm'ing it.
