Notes:

The flail mitral leaflet(s) dip-back into the left atrium during systole showing that there is a lack of attachment to the usual chordal structures. The leaflet then billows posterior to an imaginary line that could be drawn across the mitral annulus (see animation). There is inevitable severe mitral regurgitation.
