COMMENTS

The radiograph demonstrates a granular alveolar pattern diffusely visible in both lung fields. In this case, there is relatively little hilar adenopathy and no evidence of cavitation. The blunting of both costophrenic angles indicates probable small pleural effusions. The diffuse pattern is characteristic of miliary tuberculosis, although there is a broader differential which includes other alveolar processes.
