All symposium participants are required to submit a registration form.

Please download the following MS Word document, complete the required fields, and submit to iiss.info@yale.edu.

If you cannot submit the form electronically, please download the following PDF document, complete the required fields, and fax or mail it to us using the information at the bottom of this page.
