The Yale Student ID Card

The Vendor
ID Card is used only for work site identification and
access.
The ID Card consists of the following:
- Name of construction company
for which the employee works.
- The location of the work site.
- Card expiration date.

Student Affiliates
Yale College, Graduate &
Professional Schools (except Medical School)*
1. Photo ID of card recipient and
2. Spouse or same sex domestic partner's Yale ID card
or ID number and
3. Authorization to Issue ID Card form (issued by Office
of the Registrar of appropriate Yale school after receiving
the appropriate information listed below under marital
or civil union status documentation to the registrar).

for further instructions.

Affiliates
1. Photo
ID of card recipient and
2. Spouse or same sex domestic partner's Yale ID card
or ID number and
3. Document to show either marital or same sex domestic
partner status, see below. Marital

a. Copy of marriage certificate or town civil
union document
b. J2 or other spouse visa, if passport contains both
spouses' names or
c. Family Attachment Form issued by Yale University
Office of International Students and Scholars or
d. Letter signed by employee's department chair or director
on departmental letterhead attesting to marital status
or
f. Employee's life insurance policy with spouse identified
specifically as spouse

students - Affiliate ID cards will be issued at Medical
School ID enter. Please call (203) 785-4286 for information
regarding documentation.

| Uses of Yale Affiliate
ID Cards
·
Free access to Yale libraries, including borrowing and
stack privileges where such privileges are available
to employees.
· Reduced fares for weekday transportation on
the Yale Shuttle Bus.
· Free access to evening transportation by the
Yale Mini-Bus Service.
· Employee membership rates at Payne Whitney
Gymnasium and the Yale Golf Course.
· Employee discounts when offered by the Yale
RepertoryTheater.
· Access to the Yale Outdoor Recreation Center
and to the Yale Sailing Center, subject to such fees
as may be charged to employees.
· Free access to the Peabody Museum. (May not
be available during special events.)
· Discounts on purchases at the sales desks of
the Yale Art Gallery and the British Art Center.
· Employee prices for football ticket coupon
booklets (limit of 2 per family).
· Access to film society screenings, subject
to admission and membership fees.
