Divorce
If you are recently divorced or legally separated from your spouse, this qualifies as a life event and allows you to disenroll your former spouse from Yale's medical or dental coverage. You must notify the Benefits Office within 30 days from the date of the qualifying event.
The following supporting documentation is required :
- Legal separation agreement
- Court signed divorce decree
You must complete an Enrollment/Change form, as well as a Benefits Revision Request form and submit along with your supporting documentation, to the Benefits Office at 155 Whitney Ave, Room 130 or via fax at (203) 432-7575.
