Death in the family
If your spouse or dependent child has deceased, this qualifies as a life event and allows you to disenroll your deceased family member from Yale's medical or dental providers. You must notify the Benefits Office within 30 days from the date of the qualifying event.
The following documentation is required:
- Legal death certificate
You must complete an Enrollment/Change form, as well as a Benefits Revision Request form and submit along with your supporting documentation to the Benefits Office at 155 Whitney Ave, Room 130 or via fax at (203) 432-7575.
