Dental Plan Options
You can choose to enroll in dental coverage or you can waive coverage. Please note: a two- year waiting period will apply if you decide to cancel coverage after your initial enrollment. A $50 individual annual deductible and a $100 family annual deductible for restorative/specialty services will apply along with an individual annual maximum of $1,500.
