NEW GS STUDENTS

The following steps are required of all students matriculating in the Graduate School:

Bullet image By June 10: Please read the Privacy Letter and Notice of Privacy Practices. By June 30, you must complete and submit the Acknowledgement of Receipt of Notice of Privacy Practices to the office listed on the form. You may submit your Privacy Form together with the forms listed below.

Bullet image By June 30: Please read the Instructions and submit the Student Pre-Entrance Health Form and Vaccination Record for Incoming Students to the office listed on the form. You must complete the vaccination record and show proof of required immunizations to receive your Yale ID and to register at Orientation.

Bullet image By September 15: Submit any Requests for waivers of Hospitalization/Specialty Care and/or Prescription Plus Coverage. Please see the information at right before waiving any coverage. Note-New Online Waiver form will be on UHS website in summer 2008.

Bullet image For students under the age of 18 at the time of matriculation, please submit the Authorization for Medical Care of Minors to the office listed on the form.

HEALTH CARE COVERAGE OPTIONS AND HEALTH AWARD

The University provides health care for graduate students and their families through the Yale Health Plan (YHP). All enrolled students attending Yale at least half time and working towards a Yale degree automatically receive YHP primary care services at no charge. Students do not need to enroll at YHP or pay additional fees to obtain this coverage, which is called YHP Basic. Your status as a Yale University graduate student automatically makes you a student member of the health plan. Non-degree students may purchase membership in YHP as "billed associates."

If you are eligible for YHP Basic, you are automatically enrolled in YHP Hospitalization/Specialty Plan. Eligible Ph.D. students receive a Health Award that covers the full cost of single-student Yale Health Plan Hospitalization/ Specialty Coverage, half the cost of two-person coverage, and the full cost for dependent children. For questions about the Health Award, please contact the Office of Financial Aid. To enroll your dependents (spouse/same sex domestic partner
and/or children), use this form.

Enrollment in YHP Hospitalization/ Specialty Coverage automatically enrolls you in YHP Prescription Plus, as well. If you do not wish to have this prescription coverage and pay the associated premium fee, you must waive the Prescription Plus coverage by submitting the appropriate form to YHP Member Services by September 15 each academic year.

If you have other health insurance or coverage, either by being a dependent on someone else’s plan (such as a parent, spouse or domestic partner) or purchasing other coverage on your own, you may waive Yale Health Plan's Hospitalization/ Specialty Coverage by submitting the appropriate form to Member Services by September 15 each academic year. You will still be able to use YHP specialty services, but your non-Yale insurer will be billed for these services.

In addition, students have the option of enrolling in a dental and vision plan.
