Geriatric Assessment Program Mission Statement The Geriatric Assessment Program (GAP) provides basic medical and emotional support to Connecticut's elderly regardless of health, ethnicity, religion, or economic status. GAP supplements medical care through basic assessment techniques. This includes the recording of blood pressures, respiratory rates, pulses, and current medicines. In addition, GAP educates the elderly through a series of informative pamphlets covering topics as diverse as hypertension and nutrition. GAP also administers to the emotional needs of the elderly through regular visitations. Volunteers bond with their elderly residents and maintain close contact throughout the year discussing everything from the residents' medical updates to their personal lives. |
