NEW: Claims Handling Process Effective 7/1/2009
Effective July 1, 2009, Yale University will no longer be handling new workers' compensation claims internally. Any claims reported to the Office of Risk Management on or after July 1, 2009 will be handled by PMA Management Corporation. Their office is located in Meriden, CT and their contact information can be found here.
Note: All claims reported to the Office of Risk Management prior to July 1, 2009, will continue to be handled by Risk Management.
Claim Reporting
When a work-related accident occurs, the employee should notify his or her supervisor immediately. The supervisor will report it to the Office of Workers' Compensation via the Employee First Report of Injury Form.
Medical Treatment
Yale has a managed care plan for workers’ compensation claims. For non-emergency medical care, the employee must choose a physician from the Managed Care Plan directory. Failure to obtain treatment from a physician in the plan may result in denial of the claim.
During normal working hours, there are two main locations for initial treatment, Yale University Employee Health Services, or St. Raphael’s Occupational Health Plus Center.
If the employee wishes to seek treatment at University Health Services, he or she should contact the Employee Health Department at 203.432.7879 to schedule an appointment. The supervisor must supply the employee with a University Health Services Appointment Form for the initial evaluation at University Health Services.
If the employee wishes to seek treatment at St. Raphael’s Occupational Health Plus, no appointment is necessary, but it is helpful for the employee to call 203.789.3721 and advise St. Raphael’s Occupational Health Plus that he or she is on the way there for treatment. The supervisor must supply the employee with a St. Raphael's Occupational Health Plan Authorization for Injury Care form for initial evaluations.
For emergency situations, or during off hours, the employee is allowed to treat at University Health Servces Urgent Care, or the Yale New Haven Hospital or St. Raphael's Hospital emergency rooms, but any follow up care should be provided by a physician in the managed care network.
