Below are links to forms that must be submitted whenever there is an accident or incident that needs to be reported to Risk Management. Please complete the proper form and return it via fax or e-mail to the address/fax number listed on the form. All forms must be submitted within 24 hours of the accident or incident.
The Office of Risk Management recommends forms be submitted even if it is not believed at the time of the accident or incident that there were any injuries or damage. Reporting the accident or incident immediately allows The Office of Risk Management to perform a timely investigation of the facts and circumstances surrounding an accident or incident, in case injury or damage is discovered later.
