Accident and Incident Forms

Effective October 1, 2011, we have amended the process for reporting accidents and claims.  All reports should now be made on line directly to PMA Management Corporation, the company who handles claims on behalf of the University. Please do not submit any forms to the Office of Risk Management.  Complete instructions on how to report a claim can be found here. Once you access the reporting website, please enter User Name: 9841305 and Password: newclaim.

Although forms are no longer required, you are welcome to use the forms to gather the necessary information prior to filing the on line report.

The Office of Risk Management recommends reports be submitted even if it is not believed at the time of the accident or incident that there were any injuries or damage. Reporting the accident or incident immediately allows PMA and the Office of Risk Management to perform a timely investigation of the facts and circumstances surrounding an accident or incident, in case injury or damage is discovered later.  Please also file a report if the incident was a "near miss".  That will help to better evaluate trends and prevent future incidents.

Deductibles are charged to departments for property and automobile claims.  Please contact Phil Cundiff for more information on the deductible process, or click here

Note: Effective 7/1/2011, claims that are reported more than 90 days after the incident occurs or the damage is first discovered may not be covered by our insurance.  Late reporting may cause the department to be responsible for all costs.  Please file reports on a timely basis.
