MATERIAL TO BE SHIPPED (Substance for Classification)
Have any pathogens been neutralized/inactivated?
  • Is it known not to contain infectious substances?
  • Are all microorganisms present non-pathogenic for humans and/or animals?
  • Is it a dried blood spot/fecal occult blood test?
  • Is it an environmental sample, e.g. food or water that is not considered to pose a significant health risk?
  • Is it for transplant or transfusion?
YES TO ANY
NO
Does it meet the definition of an Infectious Substance, Category A?
  • Is it a cultured or amplified human pathogen?
  • Is it a Select Agent or Risk Group 4 Agent or genetic derivative of these agents?
  • Is it a Genetically Modified Organism or Genetically Modified Microorganism that meets the definition of an infectious substance (e.g. recominant VSV lab strains)?
Does Shipment Contain Dry Ice?
YES
YES TO ANY
NO
  • Is it a patient specimen for which there is only a minimal likelihood that pathogens are present?
  • It is NOT a Genetically Modified Microorganism that is considered "defective" but can still "alter a cell" (e.g. defective pathogen vectors, such as adenovirus, herpesvirus, retrovirus, AAV)?
NO Training required: Shipping Dry Ice This is a mandatory course designed to fulfill performance-specific training requirements for employees sending, transporting, or receiving dry ice with no other hazardous materials. Retraining is required every three (3) years. Click for link to training.
NO YES
UN 2814 Infectious Substance, affecting humans; or UN 2899 Infectious Substance, affecting animals UN 3373 Biological Substance, Category B Subject to "Exempt Human Specimen" or "Exempt Animal Specimen" provisions Not subject to the provisions of the "DGR" unless meeting the criteria of another class or division of the "DGR" (i.e. dry ice)

Infectious Substance, Category A Training This is a mandatory course designed to fulfill performance specific training requirements for employees who do any of the following: package, label, ship, prepare shipping documents, offer packages of hazardous materials to carriers for shipment, transport and/or receive infectious substances. This training will also fulfill the requirements for shipping materials classified as Biological Substance, Category B, Exempt Human and Animal Specimens, Dry Ice and the General Awareness training. Retraining is required every three (3) years. Click for link to training. Category B and Exempt Human or Animal Specimen Training Note: Requirement may also be fulfilled by taking the Infectious Substance, Category A Training. Biological Substances, Category B are diagnostic or clinical speciments from humans or animals that are known to harbor a pathogen or have a high probability of containing a pathogen. Exempt Human or Animal Specimens are biological substances not known to harbor a pathogen or have a lower probability of containing a pathogen. This is a mandatory course designed to fulfill performance-specific training requirements for employees who do any of the following: package, label, ship, prepare shipping documents, offer packages of hazardous materials to carriers for shipment, transport and/or receive biological substances. This training also fulfills the requirements for shipping dry ice and the General Awareness training. Retraining is required every three (3) years. Click for link to training. *DGR = Dangerous Goods Regulations issued by the US Department of Transportation (DOT) and/or the International Air Transport Association (IATA)
