Principal Investigators Responsibilities and Requirements

Principal Investigator Biosafety Requirements
P.I. Orientation to Yale's Biological Safety Manual, Procedures & Policies Training
This training focuses on Principal Investigator requirements for registering work and training employees, occupational health requirements when working with biohazardous materials, incident response, protocol risk assessment, decontamination procedures, PPE requirements and shipping procedures. The University's Training Management System will notify you if you need to complete this requirement.

II. Form 01 Initial & Triennial Registration

All Yale Principal Investigators are required to complete an update of work with potentially hazardous biological material on an annual basis. A 10-page registration document, the Form 01, and an annual update form have been developed to assist you. Completed forms are reviewed for compliance with registration and annual training requirements specified by the Occupational Safety and Health Administration (OSHA), the National Institute of Health (NIH), and the Connecticut Department of Public Health. EHS will assist you with the required update during the annual biological and chemical safety lab inspection of your laboratory.

Principal Investigators who have not completed a Form 01 or an annual update need to complete the appropriate form listed here:

Form01 Registration for the Use of Biological Materials
Form 01 Annual Registration Update for the Use of Biological Materials

or contact your Safety Advisor or EHS for a copy of the form.

Note: Completion of the Form 01 and required registrations will help satisfy many granting agency requirements for biological safety.
