How to Complete the Form

Use this form to request single or multiple copies of the Directory for delivery to the same campus address. Complete a separate order form for each campus address.

Orders must be charged to a University account and must be authorized by a business manager or other authorized department representative.
RIS Mail Services will distribute the additional copies. Click on the Submit button to send the order directly.
If you want to send the completed form by FAX or Interoffice mail:
- Print the completed form
- FAX the form to John Poitras at 432-6543
- or send to, Directory Orders, RIS Printing Service, 155 Whitney Avenue

Completing the Form

Enter the number of copies you want to order in the Number of Copies field. The total automatically calculates.
Enter the complete charging instructions for the department accounts to which you want to charge the order. Use the following format:
0000000 . 00000000 . 000000 . 000000 . 000000
Enter the name of the person who is responsible for authorizing the order, as well as the department, and phone number of that person in the appropriate fields.
Enter the date the form was completed in the Date field.
Enter the name of the contact person to whom the order should be sent, the department of that person and the campus address where the order should be sent in the appropriate fields.
