Self-Reported Grade Sheet Guidelines

If an employer requests an unofficial transcript, you may create a self-reported grade sheet as your unofficial transcript (sample below). You must create this document yourself and upload it to the system. Type your self-reported grade sheet in Microsoft Word using the information on your most recent academic record. Upload your document to eRecruiting as an unofficial transcript.

Upon submitting your self-reported grade sheet you are attesting that all information listed is accurate and true. Care and accuracy are essential in the reporting of your grades. Employers may ask that an official transcript be sent to them at a later date, so there should be no discrepancies. UCS has established procedures and regulations as stated in the Undergraduate Regulations: XV, A:

Infractions include, but are not limited to, the following: altering in any manner a transcript, associated documents, or test scores; signing up for an employment interview when not authorized to do so; purporting to be another person or utilizing a document of another person; sequestering or removing materials without permission. Infractions may be referred to the Yale College Executive Committee with a request that disciplinary action be taken.

Violations of these standards may cause revocation of the privilege to use UCS programs and facilities up to and including suspension from Yale College.
