Custom's Declarations Forms

All international and APO/FPO metered and stamped mail 10 oz. or more must have a Customs Declarations form on it.

Use PS Form 2976 for packages 10 oz. to 4 lbs.
For packages larger than 4 lbs., use PS Form 2976-A, a 4 part form

All domestic stamped mail 16 oz. or more going out of state must be kept separate for pick-up. Do not mix this mail with your outgoing U.S. mail. It must be presented separately by Campus Mail to the U.S. Postal Service. If it is mixed with your outgoing mail, it will be returned to you by the U.S. Postal Service.

The U.S. Postal Service is no longer accepting old, paper Customs Declarations Forms. Please discard all forms with a date older than January, 2004. Packages sent with forms older than January, 2004 will be returned to sender.
